2 Forms Of ID
You will need to present 2 forms of ID.
- Photo ID: Driver's license, passport, or government-issued ID with your picture.
- Proof of address: The second form of ID often needs to verify your current address, like a utility bill, lease agreement, or vehicle registration.
STEP 2:
Complete USPS 1583
This form gives us permission to receive your mail. It must be either:
- Signed in person.
- Signed and notarized.
- Signed via Zoom.

Mailbox Contract
