2 Forms Of ID

You will need to present 2 forms of ID.

  1. Photo ID:  Driver's license, passport, or government-issued ID with your picture. 
  2. Proof of address:  The second form of ID often needs to verify your current address, like a utility bill, lease agreement, or vehicle registration. 

STEP 2:

Complete USPS 1583

This form gives us permission to receive your mail.  It must be either: 

  1. Signed in person. 
  2. Signed and notarized.
  3. Signed via Zoom.
